Compulsory Third Party (CTP)

For CTP insurance, please bring:
- Your CTP insurer and GP's details
- Letter from CTP insurer stating that your initial treatment has been approved
- All correspondences from your treating practitioner (GP/ Doctor) and CTP insurer
- Treatment Plan to your Physiotherapist to complete if required by CTP insurer
- Any related scans and reports (X-ray, ultrasound, CT scan & MRI) to your condition
DVA/Medicare EPC
